I hereby acknowledge that I am voluntarily using the facilities provided by Jamie Baker Fitness, located at Unit 17, Hampers Common Industrial Estate, Petworth, West Sussex.
I am fully aware that the use of exercise equipment, including weight machines, cardio equipment and free weights, involves risk of injury or death. I agree to assume all risks associated with my use of the gym’s facilities and equipment.
I agree to waive any and all claims against Jamie Baker Fitness, its owners, employees, agents and affiliates, arising out of my use of the gym’s facilities and equipment. I also agree to indemnify and hold harmless the gym, its owners, employees, agents and affiliates from any and all claims, demands, damages, costs and expenses, including reasonable attorney’s fees, arising out of my use of the gym’s facilities and equipment.
I acknowledge that I have had the opportunity to inspect the facilities and equipment provided by the gym, and that I have determined that they are safe and suitable for my use. I agree to follow all safety instructions provided by the gym’s staff, and to immediately report any unsafe conditions or equipment to a staff member.
I agree that this Waiver and release of Liability shall be binding upon my heirs, executors, administrators and assigns. I understand that by signing this agreement, I am giving up certain legal rights, and that I sign this agreement voluntarily and of my own free will.
I have read this Waiver and Release of Liability, and fully understand its terms. I understand that I am giving up substantial rights by signing this agreement, and I sign it voluntarily without any inducement or assurance of any nature.
